The intermediate care service is a REFERRAL ONLY SERVICE that exists to provide care to people in their own home including residential care. The aims of the service are:
* To prevent unnecessary admission to hospital or long term care
* To support people following discharge from hospital
* To provide community rehabilitation
The intermediate care team consists of assistants/support workers, Occupational Therapists, Physiotherapists, Therapy Assistants, Assessment and Review co-ordinators, and Pharmacists.
Who is eligible?
People 18 years and over who require rehabilitation.
People who are medically stable who would otherwise require hospital admission.
People with reduced independence.
People with reduced confidence.
People who can safely receive intravenous antibiotics at home.
To access the service:
Referral direct from a hospital ward, from any of the specialities within the Hospital Trusts.
From GP, District Nurse, Community Matron or emergency care practitioner
Local authority Occupational Therapist or Care Managers.
