The South Gloucestershire Access Team take referrals for clients of working age, who have severe and enduring mental illness and who are difficult to engage with services and/or those who have complex rehabilitation needs. The team works with them towards recovery, in their own homes and communities. Referrals are accepted from a wide range of agencies, but when existing psychiatric and statutory services are involved, it is expected that a referral would originate from them. Telephone support available out of hours.
The team is able to offer a wide range of social, psychological and medical interventions, due to its multi-agency, multi-disciplinary nature. All members of the team receive regular supervision, support and training. There is a particular emphasis on safety, aiming to minimise the risks of self-harm, harm to others, vulnerability and abuse.
The team aims to offer a client-led service which caters for the clients' perceived needs, and not necessarily that of the service, and helps them on their journey to recovery.
