Royal Voluntary Service helping to deliver Home From Hospital NHS Surge Support

The Royal Voluntary Service are supporting the NHS to address the challenges faced when patients are ready to be discharged from hospital. They will be helping the Home from Hospital teams by providing two support packages. An assisted discharge package providing up to 72 hrs support; and a longer 4 week Community Wrap Around Support package.

The assisted discharge support could include collecting patients from ward / discharge lounge and providing them with volunteer transport home. They would ensure they are settled, safe, warm and have food and medication. This could be followed by onward assessment of needs and actively signposting to other local providers.


Community Wrap Around Support would provide ongoing community-based support for patients once they have returned home. They could provide regular check ins in person and via telephone for up to 4 weeks. And also help with an onward assessment of needs, planning support and recovery goals.
